Oleic acid is a monounsaturated omega-9 fatty acid, most commonly found in olive oil, and has been researched for its effects on metabolic health, including Non-Alcoholic Fatty Liver Disease (NAFLD). Scientific interest in oleic acid for NAFLD primarily comes from epidemiological and interventional studies of the Mediterranean diet, which is rich in oleic acid due to high olive oil consumption. Experimental research indicates that oleic acid may improve lipid metabolism, reduce hepatic fat accumulation, and exert anti-inflammatory effects in animal models of fatty liver disease. Some clinical studies suggest that diets high in monounsaturated fats, particularly from olive oil, can reduce liver fat content and improve markers of liver function in humans with NAFLD. However, the evidence is indirect; benefits are associated with dietary patterns rather than isolated supplementation of oleic acid. There are few, if any, randomized controlled trials on pure oleic acid supplementation as a therapy for fatty liver disease. Thus, while there is scientific rationale and some supporting evidence for oleic acid-rich diets benefiting fatty liver disease, the direct evidence for oleic acid itself is limited, and overall, the strength of evidence is moderate at best.
