Oleoylethanolamide (OEA) is a naturally occurring lipid mediator that has been investigated for its effects on lipid metabolism, appetite regulation, and liver health. Several preclinical studies have shown that OEA can reduce hepatic fat accumulation, improve insulin sensitivity, and decrease inflammation in models of non-alcoholic fatty liver disease (NAFLD). The proposed mechanisms include activation of peroxisome proliferator-activated receptor-alpha (PPAR-α), which increases fatty acid oxidation and reduces lipogenesis, as well as modulation of appetite and body weight. Animal studies have demonstrated that OEA supplementation can ameliorate steatosis and liver damage in rodents with diet-induced fatty liver disease. However, direct human clinical research on OEA for the treatment or support of fatty liver disease is limited. Most evidence comes from animal models and in vitro studies. There is some indirect support from studies in humans showing OEA's effects on lipid metabolism and weight management, but well-controlled clinical trials in patients with NAFLD or NASH are lacking. Therefore, while there is a plausible scientific rationale and encouraging preclinical data, the current level of evidence supporting the use of OEA for fatty liver disease remains limited and preliminary. Further human studies are required to confirm efficacy and safety for this specific indication.
