Phospholipids, particularly those derived from soy (such as phosphatidylcholine), have been investigated for their potential role in the management of Fatty Liver Disease (FLD), including non-alcoholic fatty liver disease (NAFLD) and alcoholic liver disease. The rationale for their use is based on the crucial role phospholipids play in maintaining cell membrane integrity, supporting lipid metabolism, and facilitating the export of triglycerides from the liver. Some clinical trials, particularly in Eastern Europe and Asia, have suggested that polyenylphosphatidylcholine (PPC) supplementation may improve liver enzyme levels, reduce hepatic steatosis, and slow fibrosis progression in patients with fatty liver disease. However, these studies often have methodological limitations, such as small sample sizes, short durations, or lack of rigorous controls. Meta-analyses and systematic reviews indicate that while there is some evidence for benefit, it remains limited and not definitive.

Major clinical guidelines (e.g., AASLD, EASL) do not currently recommend phospholipid supplementation as standard therapy for fatty liver disease due to insufficient high-quality evidence. Most support for their use comes from preliminary trials, historical use in certain regions, and mechanistic plausibility rather than robust, large-scale randomized controlled trials. Therefore, while there is a scientific basis and some clinical investigation, the overall evidence is weak to moderate, justifying a rating of 2 out of 5.
