Phthalides are a class of bioactive compounds found mainly in celery (Apium graveolens) and some other plants. Their use for supporting or treating fatty liver disease (also known as non-alcoholic fatty liver disease, NAFLD) is primarily justified by emerging scientific evidence rather than traditional medicine. Several preclinical studies have investigated phthalides, especially butylphthalide, for their hepatoprotective effects. Animal studies indicate that phthalides can reduce hepatic steatosis, lower serum lipid levels, and improve antioxidant status in the liver. The mechanisms proposed include modulation of lipid metabolism, reduction of oxidative stress, and anti-inflammatory effects.

However, most of the available research is limited to animal or in vitro models, and robust human clinical data is lacking. There are no major clinical guidelines that recommend phthalides as a treatment for fatty liver disease, and their use in this context is not yet considered evidence-based medicine. Therefore, while scientific interest exists and preliminary results are promising, the overall strength of evidence is still weak (rated as 2/5). More well-designed human studies are needed before phthalides can be confidently recommended for NAFLD in clinical practice.
