Polyunsaturated fatty acids (PUFAs), particularly omega-3 fatty acids such as eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), have been investigated for their role in managing non-alcoholic fatty liver disease (NAFLD). Scientific studies suggest that PUFAs may reduce liver fat accumulation and improve lipid profiles. Mechanistically, omega-3 PUFAs are thought to modulate hepatic gene expression, decrease hepatic lipogenesis, increase fatty acid oxidation, and exert anti-inflammatory effects, which collectively may help reduce hepatic steatosis and slow NAFLD progression.
Clinical evidence is moderate. Meta-analyses and randomized controlled trials have shown that supplementation with omega-3 PUFAs can significantly reduce liver fat content in people with NAFLD, though effects on liver enzymes (ALT, AST) and histological endpoints are inconsistent. For example, a 2016 meta-analysis in Clinical Nutrition found that omega-3 supplementation reduced liver fat but did not significantly improve liver histology. Guidelines from the American Association for the Study of Liver Diseases (AASLD) and European Association for the Study of the Liver (EASL) acknowledge the potential benefit of omega-3 PUFAs for reducing hepatic steatosis, but do not recommend them as standard therapy due to variable results and limited long-term data.
Overall, there is moderate scientific validation for the use of polyunsaturated fatty acids in supporting the management of fatty liver disease, particularly for reducing liver fat, but further research is needed to establish robust clinical recommendations.
