Reishi mushroom (Ganoderma lucidum) has been traditionally used in East Asian medicine for its supposed hepatoprotective, anti-inflammatory, and immunomodulatory properties. In recent years, there has been growing scientific interest in its potential benefits for liver health, including the management of non-alcoholic fatty liver disease (NAFLD). Preclinical studies in animal models and in vitro experiments suggest that reishi mushroom extracts may reduce hepatic lipid accumulation, improve liver enzyme profiles, and attenuate inflammation and oxidative stress — key factors contributing to fatty liver disease. For instance, polysaccharides and triterpenoids from reishi have demonstrated anti-lipogenic and antioxidant effects in rodent models fed high-fat diets, which mimic fatty liver disease in humans.

However, clinical evidence in humans remains limited. A small number of clinical trials have evaluated reishi’s effect on liver enzymes or metabolic parameters, but few have directly assessed its impact on NAFLD or fatty liver in a robust, controlled manner. The quality and quantity of human data are insufficient to draw firm conclusions or to recommend reishi mushroom as a primary or adjunctive therapy for fatty liver disease. Thus, while there is a clear scientific rationale for further investigation and some promising preclinical data, the current scientific evidence supporting the use of reishi mushroom for fatty liver disease is weak to modest (rated 2/5).
