Tauroursodeoxycholic acid (TUDCA) is a bile acid derivative that has been studied for its potential hepatoprotective effects, including use in the context of fatty liver disease (nonalcoholic fatty liver disease, NAFLD). Scientific interest in TUDCA for NAFLD is based on its ability to reduce endoplasmic reticulum (ER) stress, improve insulin sensitivity, and promote bile acid homeostasis in the liver. Animal studies have shown that TUDCA can decrease liver fat accumulation, inflammation, and fibrosis markers in models of NAFLD. Small-scale human trials have demonstrated improvements in liver enzymes (e.g., ALT, AST) and certain metabolic parameters, but evidence of its impact on liver histology or long-term outcomes is limited.

A 2012 randomized controlled trial (RCT) in humans (Kars et al.) found no significant benefit of TUDCA on hepatic steatosis after 6 months, though some metabolic improvements were noted. More recent studies and meta-analyses highlight that while TUDCA is well-tolerated and biologically plausible as a treatment, robust clinical data—particularly from large, long-term RCTs—are lacking. Overall, TUDCA is supported by preclinical evidence and limited clinical data for fatty liver disease, but it is not yet an established or first-line therapy. Thus, its use is considered experimental, with moderate but inconclusive scientific validation as of 2024.
