Trans-pterostilbene is a natural polyphenol compound structurally similar to resveratrol, found in blueberries and Pterocarpus marsupium. There is emerging scientific interest in its potential for supporting fatty liver disease (also known as non-alcoholic fatty liver disease, NAFLD), though the evidence is still preliminary and mostly based on animal and cell studies.
Several preclinical studies have demonstrated that trans-pterostilbene exerts anti-inflammatory, antioxidant, and lipid-lowering effects in rodent models of NAFLD. For example, research published in Phytotherapy Research and Journal of Nutritional Biochemistry has shown that pterostilbene supplementation can reduce hepatic fat accumulation, improve insulin sensitivity, and decrease markers of oxidative stress in mice fed a high-fat diet. The proposed mechanisms include modulation of lipid metabolism genes, enhancement of antioxidant enzyme activity, and suppression of pro-inflammatory pathways. In vitro studies in hepatocyte cell lines support these findings, suggesting pterostilbene can reduce fat buildup and oxidative damage at the cellular level.
However, there is currently a lack of robust human clinical trials directly evaluating trans-pterostilbene for fatty liver disease. Most evidence is limited to animal models, and no large-scale, randomized controlled trials in humans have confirmed its efficacy or safety for this indication. Therefore, while scientific rationale exists, the overall evidence supporting its use for fatty liver disease remains limited (rated 2 of 5), and further clinical research is needed before it can be recommended as a standard therapy.
