Tributyrin, a triglyceride form of butyric acid, has garnered interest for its potential benefits in metabolic and liver health, including the management of non-alcoholic fatty liver disease (NAFLD). The scientific rationale is primarily based on preclinical studies. Tributyrin acts as a prodrug for butyrate, a short-chain fatty acid that influences gut-liver axis signaling, inflammation reduction, and lipid metabolism. In animal models, tributyrin supplementation has demonstrated protection against hepatic steatosis, inflammation, and fibrosis by modulating key pathways such as AMPK activation, inhibition of NF-κB signaling, and enhancement of mitochondrial function. Some studies have shown that tributyrin improves insulin sensitivity and decreases hepatic fat accumulation, both relevant to NAFLD pathology. However, direct human clinical evidence is limited. A few small-scale clinical trials have explored butyrate or tributyrin supplementation for metabolic health, showing modest improvements in markers of liver function and inflammation, but these results remain preliminary. The bulk of current support comes from rodent studies and mechanistic insights rather than large, controlled human trials. Thus, while there is a compelling scientific basis and early evidence supporting tributyrin’s use for fatty liver disease, it cannot be considered a well-established therapy at this time. Ongoing research and clinical trials will be crucial in clarifying its efficacy and safety in humans.
