Urolithin A is a metabolite produced by gut microbiota from ellagitannins found in foods like pomegranates and walnuts. Its potential benefits for fatty liver disease (non-alcoholic fatty liver disease, NAFLD) have been investigated primarily in preclinical (animal and cell) studies. These studies suggest Urolithin A may improve mitochondrial function, reduce oxidative stress, and decrease hepatic lipid accumulation—mechanisms relevant to the pathogenesis of fatty liver disease. For example, a 2021 study in Nature Communications demonstrated that Urolithin A supplementation reduced hepatic steatosis and inflammation in mouse models of NAFLD. Another study published in Cell Reports Medicine in 2022 noted that Urolithin A promoted mitophagy and improved cellular energy metabolism in liver tissues.
However, direct clinical evidence in humans with fatty liver disease is currently lacking. Most of the supportive data come from animal models or in vitro studies, and human trials have so far focused more on metabolic health, muscle function, and aging rather than specifically on NAFLD. There are ongoing clinical trials evaluating Urolithin A’s safety and efficacy in various metabolic conditions, but definitive evidence for its use in fatty liver disease is not yet established.
In summary, while preclinical data are promising and provide a plausible mechanistic rationale, the evidence for Urolithin A’s use in fatty liver disease remains preliminary. Its rating is therefore a 2, pending future human studies.
