Vitamin E has been studied for its effects on nonalcoholic fatty liver disease (NAFLD), particularly in the form of alpha-tocopherol. Most major clinical trials—such as the PIVENS trial—have used alpha-tocopherol, not mixed tocotrienols. These trials demonstrated that vitamin E supplementation (800 IU/day) improved liver histology and reduced inflammation in non-diabetic adults with biopsy-proven nonalcoholic steatohepatitis (NASH), a severe form of fatty liver disease. As a result, both the American Association for the Study of Liver Diseases (AASLD) and the European Association for the Study of the Liver (EASL) suggest considering vitamin E for non-diabetic adults with confirmed NASH.

However, mixed tocotrienols (another form of vitamin E) have been less studied. Limited small-scale clinical trials—primarily from Southeast Asia—have suggested potential benefits of tocotrienols in improving liver enzymes and reducing markers of oxidative stress in NAFLD patients, but these studies are few and often lack the rigorous design and large sample sizes seen in alpha-tocopherol trials. Mechanistically, vitamin E's antioxidant properties are believed to reduce oxidative stress and inflammation in liver tissue, which are key processes in NAFLD progression.

In summary, vitamin E (specifically alpha-tocopherol) has moderate scientific evidence supporting its use in selected NAFLD/NASH patients. Evidence for mixed tocotrienols is promising but still preliminary and not yet established in clinical guidelines.
