Vitamin E, particularly in the form of alpha-tocopherol, has been studied for its potential benefits in nonalcoholic fatty liver disease (NAFLD), a spectrum that includes nonalcoholic steatohepatitis (NASH). Several randomized controlled trials (RCTs), including the PIVENS trial (Sanyal et al., 2010), have shown that high-dose Vitamin E (800 IU daily) can improve certain histological features of NASH in non-diabetic adults. The proposed mechanisms include Vitamin E’s antioxidant properties, which may help counteract oxidative stress implicated in the progression of fatty liver disease.

However, the evidence is more robust for alpha-tocopherol than for tocotrienols, and most major clinical trials have used the former. There is emerging but limited preclinical and early-phase clinical research on tocotrienols, suggesting potential hepatoprotective and anti-inflammatory effects, but these findings are not yet sufficient for strong clinical recommendations. Clinical guidelines (such as those from the American Association for the Study of Liver Diseases) cautiously endorse Vitamin E for select non-diabetic adults with biopsy-proven NASH, but not for all forms of fatty liver disease or for those with diabetes due to potential safety concerns.

In summary, while there is moderate scientific evidence supporting Vitamin E (primarily alpha-tocopherol) for certain NAFLD patients, the evidence for tocotrienols is weak and largely limited to preliminary studies.
