Vitamin E, particularly in the form of tocotrienols and tocopherols, has been investigated for its use in treating non-alcoholic fatty liver disease (NAFLD). The rationale stems from its antioxidant properties, which may help reduce oxidative stress involved in the progression of NAFLD. Most clinical studies on vitamin E and fatty liver disease have used alpha-tocopherol (the most common form of vitamin E), and some randomized controlled trials (RCTs) have shown modest improvements in liver histology and biochemical markers in non-diabetic adults with non-alcoholic steatohepatitis (NASH). For example, the PIVENS trial (NEJM, 2010) found that 800 IU/day of alpha-tocopherol improved liver histology in non-diabetic patients with NASH compared to placebo. Tocotrienols, a less common form of vitamin E, have shown promise in preclinical studies and limited human trials. Some small studies suggest tocotrienols may reduce liver fat, inflammation, and fibrosis markers, potentially more effectively than tocopherols due to their greater antioxidant activity. However, most guidelines (such as AASLD and EASL) recommend vitamin E (tocopherol) only in specific populations (non-diabetic, biopsy-proven NASH) and emphasize that evidence for tocotrienols is still emerging and limited by small sample sizes and short durations. In summary, there is moderate scientific evidence (not tradition) supporting vitamin E for certain NAFLD patients, primarily with tocopherol, and much less robust but emerging evidence for tocotrienols.
