Vitamin D has been investigated for its potential role in managing fibromyalgia syndrome due to the observed overlap of symptoms such as muscle pain and fatigue with those seen in vitamin D deficiency. Several observational studies have reported that individuals with fibromyalgia often exhibit lower serum vitamin D levels compared to healthy controls. Some small randomized controlled trials (RCTs) and open-label studies have evaluated the effect of vitamin D supplementation on fibromyalgia symptoms. Results have been mixed: while a few studies noted modest improvements in pain and quality of life among those with documented deficiency who received vitamin D supplementation, others did not find significant benefits compared to placebo. Meta-analyses and systematic reviews published up to 2023 generally conclude that the current evidence is weak and inconsistent, with most studies limited by small sample sizes, short durations, and methodological differences. There is no high-quality evidence to support widespread vitamin D supplementation for all fibromyalgia patients, but supplementation may be considered in those with proven deficiency. As such, vitamin D is not a standard or primary treatment for fibromyalgia, but it may have a supportive role in selected cases. Further large, well-designed RCTs are needed to clarify its efficacy in this condition.
