Bile acids are endogenously produced compounds that play a crucial role in the digestion and absorption of dietary fats. Supplementing with bile acids, such as ox bile, is a practice primarily used to support individuals with compromised gallbladder function or those who have undergone cholecystectomy (gallbladder removal). The scientific rationale is based on the fact that the gallbladder stores and concentrates bile, releasing it into the small intestine to emulsify fats. When the gallbladder is not functioning properly or is absent, bile flow may be insufficient, leading to impaired fat digestion, steatorrhea, and deficiencies in fat-soluble vitamins. Several clinical studies and reviews have validated the use of bile acid supplements in conditions such as bile acid insufficiency and post-cholecystectomy syndrome. For example, supplementation with exogenous bile acids can improve fat digestion and reduce gastrointestinal symptoms in affected patients (World Journal of Gastroenterology, 2009; Hepatology, 2013). While there is robust support for the use of bile acids in individuals with specific medical needs, the evidence is less clear for the routine use of bile acids in healthy individuals with normal gallbladder function. Nonetheless, the scientific basis for supplementation in cases of gallbladder dysfunction or absence is well established, justifying a high evidence rating.
