Bile salts are scientifically validated for use in individuals with a sluggish gallbladder (biliary dyskinesia) or those who have had their gallbladder removed (cholecystectomy). The gallbladder stores and concentrates bile, releasing it in response to fatty meals. When the gallbladder is removed, bile flows directly from the liver into the intestine, but the release is less regulated, sometimes leading to impaired fat digestion and symptoms such as steatorrhea (fatty stools), bloating, and diarrhea.
Supplemental bile salts (often in the form of ox bile or purified bile acids) can help compensate for reduced or uncoordinated bile flow. Clinical guidelines and reviews (for example, World Journal of Gastroenterology, 2015) support the use of bile acid supplements to improve fat absorption and reduce gastrointestinal symptoms after cholecystectomy. Additionally, in cases of biliary insufficiency where bile flow is sluggish, bile salts may aid in emulsifying dietary fats and facilitating absorption of fat-soluble vitamins (A, D, E, K).
While there is robust physiological rationale and some clinical evidence supporting bile salt supplementation, especially after gallbladder removal, high-quality randomized controlled trials are limited. However, the practice is widely accepted in gastroenterology for selected patients, particularly those with persistent symptoms. Patients should consult a healthcare provider before starting bile salts, as inappropriate use can cause gastrointestinal discomfort or diarrhea.
