European Barberry (Berberis vulgaris) has a long history of use in traditional herbal medicine, particularly in Europe and the Middle East, for a range of digestive complaints, including disorders of the liver and gallbladder. Traditional herbalists have used barberry preparations to stimulate bile flow, which was believed to help with "sluggish" gallbladder function or in cases where the gallbladder has been removed (post-cholecystectomy), to aid digestion and reduce symptoms such as bloating or indigestion. The key active constituent, berberine, is thought to be responsible for these effects, as it may promote bile secretion in animal models. However, scientific studies specifically examining barberry or berberine in humans with gallbladder dysfunction or after gallbladder removal are lacking. Some preclinical (animal and in vitro) research suggests berberine can stimulate bile flow and improve certain markers of liver and gallbladder function, but controlled clinical trials in humans are absent. Thus, while its use is well supported by tradition and there is a plausible mechanism of action, modern scientific validation for its effectiveness and safety in this specific context is limited.
