Lipase supplementation is scientifically supported for individuals with gallbladder issues, particularly after cholecystectomy (gallbladder removal), but the evidence is moderate rather than robust. The gallbladder stores and concentrates bile, which emulsifies dietary fats to aid digestion. When the gallbladder is sluggish or removed, bile flow into the small intestine can be less efficient or less synchronized with meals, sometimes resulting in fat malabsorption, bloating, or diarrhea. Lipase is an enzyme produced mainly by the pancreas that breaks down dietary fat into absorbable components (fatty acids and glycerol). Supplemental lipase is intended to enhance fat digestion when endogenous (naturally produced) digestive processes are compromised.

Clinical studies and reviews indicate that pancreatic enzyme supplementation, which typically includes lipase, can improve fat absorption and reduce gastrointestinal symptoms in some patients post-cholecystectomy or with biliary insufficiency. However, most research focuses on comprehensive pancreatic enzyme products rather than isolated lipase, and the evidence is stronger in cases of pancreatic insufficiency than in gallbladder dysfunction alone. For mild fat maldigestion due to gallbladder issues, lipase may provide symptomatic relief, but it is not universally necessary or effective for all patients. Guidelines suggest enzyme therapy on a case-by-case basis.

In summary, there is scientific rationale and moderate evidence supporting lipase supplementation for some individuals with gallbladder dysfunction or after gallbladder removal, particularly when fat malabsorption symptoms are present. The strength of evidence is higher for general pancreatic enzyme blends than lipase alone, and individual response can vary.
