Ajoene is a sulfur-containing compound derived from garlic (Allium sativum) and has been investigated for its antimicrobial and anti-inflammatory properties, which are relevant to the treatment of gingivitis—a common form of gum inflammation caused by bacterial plaque. Several in vitro studies have demonstrated that ajoene exhibits significant antibacterial activity against key oral pathogens involved in gingivitis, such as Porphyromonas gingivalis and Streptococcus mutans. Its mechanism of action is believed to involve disruption of microbial cell membranes and inhibition of quorum sensing, thus reducing biofilm formation and virulence factor production.

Additionally, ajoene has shown anti-inflammatory effects by modulating the production of pro-inflammatory cytokines, which may help in reducing the inflammatory response in gingival tissues. However, clinical evidence in humans is limited. A small number of pilot studies and case reports have suggested that topical application of garlic extracts (which may contain ajoene) can reduce gingival inflammation and bleeding, but these studies are often limited by small sample sizes, lack of blinding, or insufficient controls.

In summary, while the preclinical evidence for ajoene’s antimicrobial and anti-inflammatory effects is promising and provides a scientific rationale for its use in supporting gingivitis treatment, robust clinical trials in humans are lacking. Therefore, while its use is supported by scientific principles and preliminary data, more research is needed to fully validate its efficacy and safety in this context.
