Lactoperoxidase is an enzyme naturally present in saliva and is a component of the salivary peroxidase system, which plays a role in oral antimicrobial defense. Its use in oral care products, including toothpastes and mouthwashes, is based on its ability to catalyze the oxidation of thiocyanate ions (SCN-) in the presence of hydrogen peroxide, resulting in the production of hypothiocyanite (OSCN-), an antimicrobial agent. Several in vitro studies have demonstrated that lactoperoxidase can inhibit the growth of oral pathogens associated with gingivitis, such as Streptococcus mutans and Porphyromonas gingivalis. Clinical trials examining lactoperoxidase-containing oral hygiene products have shown modest improvements in gingival health, including reductions in gingival inflammation and plaque accumulation, though results are mixed and often limited by small sample sizes and short study durations. A systematic review (e.g., Mielczarek et al., 2012) concluded that while there is some evidence to support a beneficial effect of lactoperoxidase-containing products in reducing gingival inflammation, the quality of evidence is low to moderate and not consistent across all studies. Therefore, the use of lactoperoxidase for gingivitis is scientifically justified, but the overall strength of evidence is limited, and it should be considered as an adjunct rather than a replacement for established oral hygiene practices.
