Neem (Azadirachta indica) has a long history of traditional use in oral hygiene in India and other parts of Asia, where neem twigs have been used as natural toothbrushes. This traditional use laid the groundwork for scientific interest. In recent years, several in vitro and in vivo studies have evaluated neem's efficacy for gingivitis, an inflammatory condition of the gums. Research shows that neem extracts possess antimicrobial, anti-inflammatory, and antioxidant properties, which may contribute to improved oral health and reduced dental plaque—a primary cause of gingivitis.

Clinical studies, though generally small in scale, have reported that mouthwashes and gels containing neem extract can significantly reduce plaque index, gingival bleeding, and other signs of gingivitis, performing comparably to standard treatments like chlorhexidine in some cases. The proposed mechanisms include the inhibition of pathogenic oral bacteria such as Streptococcus mutans and Porphyromonas gingivalis, as well as modulation of the inflammatory response in gum tissues.

However, the evidence base is not as robust as for conventional treatments. Most studies are short-term, have modest sample sizes, and vary in quality. Systematic reviews acknowledge neem's potential but call for larger, better-designed clinical trials to confirm its efficacy and safety for long-term use. In summary, neem has both traditional and scientific backing for its use in managing gingivitis, but current scientific evidence is moderate rather than definitive.
