Specialized pro-resolving mediators (SPMs), including resolvins, protectins, and maresins, are bioactive lipid compounds derived from omega-3 fatty acids. Their primary role in the body is to actively resolve inflammation and promote tissue healing. In the context of gingivitis—a common, reversible form of gum disease characterized by inflammation—the use of SPMs has garnered scientific interest due to their potential to modulate the host inflammatory response and promote resolution rather than simply suppressing inflammation.

Preclinical studies, including animal models of periodontitis and gingivitis, have shown that local or systemic administration of SPMs can reduce inflammatory cell infiltration, decrease tissue destruction, and enhance regeneration of periodontal tissues. For example, research has demonstrated that resolvin E1, when applied topically or injected, can decrease inflammatory markers and promote tissue repair in gingival tissues. Some small human pilot studies and clinical trials have begun to explore the effects of SPMs in oral health, with preliminary results suggesting improved clinical parameters such as reduced gingival inflammation and pocket depth.

However, the evidence base remains limited: there are few large, high-quality human clinical trials, and SPMs are not yet established as standard therapy in dentistry. The current scientific support is promising but preliminary, placing the evidence at a 2 on the scale provided. While SPMs are not used traditionally for gingivitis, their scientific rationale is grounded in modern understanding of inflammation resolution.
