The primary rationale for its use is the presence of berberine, an alkaloid compound found in the root and bark, which was observed to have broad antimicrobial activity. Traditional healers and herbalists have employed decoctions or extracts of barberry as part of remedies for sexually transmitted infections, including gonorrhea, likely due to its perceived ability to "cleanse" or "purify" the urinary tract and reduce symptoms.

However, direct scientific evidence supporting the efficacy of European Barberry specifically for the treatment of gonorrhea in humans is lacking. While in vitro studies have demonstrated that berberine possesses antimicrobial properties against various bacteria, there are no robust clinical trials or modern studies that confirm the effectiveness of barberry against Neisseria gonorrhoeae, the causative agent of gonorrhea.
