Omega-3 fatty acids, primarily found in fish oil, have been researched for their anti-inflammatory properties, which provide a theoretical basis for their use in gout, a condition characterized by acute and chronic inflammation due to uric acid crystal deposition in joints. Several small studies and animal models suggest that omega-3 fatty acids (especially EPA and DHA) may reduce the production of pro-inflammatory cytokines and decrease leukocyte migration, both of which are relevant to the inflammatory process in gout.

However, direct clinical evidence supporting omega-3 supplementation as a treatment or adjunct for gout is limited. Most studies focus on omega-3s for general arthritis or inflammatory conditions, not specifically gout. One small pilot study (2017) showed that high-dose omega-3 supplementation may lower serum uric acid levels and reduce the frequency of gout flares, but its sample size and design were not robust enough for broad conclusions. Larger, well-controlled studies are lacking.

In summary, while there is a plausible anti-inflammatory rationale and limited scientific evidence suggesting potential benefit, the use of omega-3 fatty acids for gout is not strongly supported by high-quality clinical trials. The evidence is preliminary, and omega-3s are not a substitute for established urate-lowering or anti-inflammatory medications in gout management.
