Thyroid hormone therapy is scientifically validated in the context of hair loss or thinning when the underlying cause is hypothyroidism (an underactive thyroid gland). The thyroid hormones, mainly thyroxine (T4) and triiodothyronine (T3), play crucial roles in regulating metabolism, including the growth and maintenance of hair follicles. Hypothyroidism is a well-established cause of diffuse hair thinning and hair loss, often presenting as telogen effluvium (a shift of hair follicles into the resting phase). Numerous clinical studies and endocrinology guidelines indicate that correcting thyroid hormone deficiency with levothyroxine or other thyroid hormone replacement therapy can reverse or significantly improve hair loss in affected individuals.

However, thyroid hormone therapy is not indicated for hair loss in people with normal thyroid function, and inappropriate use can cause significant adverse effects. The evidence supporting thyroid hormone therapy for hair loss is strong (rated 4/5) but applies specifically to patients with confirmed hypothyroidism. In these individuals, successful treatment of the thyroid disorder often leads to gradual improvement in hair density and quality over several months. There is no evidence supporting the use of thyroid hormones for hair loss unrelated to thyroid dysfunction, and such practice is not recommended.
