Guaranine, the principal active compound in guarana (Paullinia cupana), is chemically identical to caffeine. The use of caffeine-containing substances like guaranine for headache relief—especially in the context of tension-type and migraine headaches—has scientific backing. Caffeine acts as a central nervous system stimulant and is known to have vasoconstrictive properties, which can counteract the vasodilation associated with certain headaches. It is commonly included as an adjuvant ingredient in over-the-counter headache medications (such as those containing acetaminophen, aspirin, and caffeine).
Several randomized controlled trials and systematic reviews have demonstrated that caffeine can enhance the analgesic efficacy of other medications for acute headache treatment. For example, a 2017 Cochrane review concluded that the addition of caffeine at doses of 100 mg or more to standard analgesics modestly increases the proportion of people who achieve significant pain relief. However, evidence supporting the independent use of caffeine (or guaranine) alone for headache is less robust, and excessive or frequent use may paradoxically contribute to medication-overuse headaches or withdrawal headaches.
Although most studies reference "caffeine" rather than "guaranine" specifically, the two are functionally the same in this context. Therefore, guaranine's use for headache relief is scientifically validated, but mainly as an adjunct to other analgesics rather than as a primary or standalone intervention.
