Petasines are active compounds found in the plant Petasites hybridus (commonly known as butterbur). Butterbur extracts, standardized to contain petasines and free of toxic pyrrolizidine alkaloids, have been investigated for their role in headache management, particularly migraine prevention. Several randomized controlled trials have been conducted, with some suggesting that butterbur extracts can reduce the frequency of migraine attacks compared to placebo. For example, a notable study published in Neurology (Lipton et al., 2004) demonstrated that a specific butterbur root extract (Petadolex) significantly reduced migraine attack frequency over a 4-month period.

Systematic reviews and meta-analyses (e.g., from the American Academy of Neurology and the American Headache Society) have concluded that there is moderate evidence supporting the efficacy of butterbur in migraine prevention, warranting a rating of 3 for evidence strength. However, due to concerns about potential liver toxicity from pyrrolizidine alkaloids, only purified, PA-free extracts are recommended, and some regulatory agencies have advised caution or restricted its use. While the mechanism is not fully understood, petasines are believed to exert anti-inflammatory and antispasmodic effects that may underlie their benefit in headache prophylaxis.
