Asafetida (Ferula assa-foetida) has been used in traditional medicine systems, particularly in Ayurveda and Unani, for the relief of headaches, including those related to sinus congestion. Classical Ayurvedic texts mention asafetida as a remedy for "vatika" disorders, which can include headaches with congestion, based on its reputed ability to clear nasal passages and act as an anti-inflammatory and antispasmodic agent. The traditional method often involves inhalation of the fumes or topical application of asafetida paste to the forehead, or ingestion in small amounts for systemic effects. The rationale is that asafetida's pungent volatile oils help open blocked sinuses and reduce pressure that can contribute to headaches.

However, modern scientific studies directly examining asafetida's effectiveness for sinus headache are extremely limited. Some in vitro and animal studies indicate that asafetida has anti-inflammatory, antispasmodic, and expectorant properties, which could theoretically help with sinus congestion and associated headaches. Nevertheless, there is a lack of well-controlled clinical trials in humans validating its use specifically for sinus headache relief. Thus, the primary support for this use remains traditional, with some weak pharmacological basis but little direct scientific confirmation.
