Luo Han Guo (Siraitia grosvenorii), also known as monk fruit, is primarily used in Traditional Chinese Medicine (TCM) for its cooling properties, to relieve cough, soothe sore throat, and moisten the lungs. Its use for headaches—particularly sinus headaches—originates from its traditional indication for "clearing heat" and resolving phlegm, which are concepts in TCM sometimes associated with sinus congestion and related head pain. However, there is minimal direct historical documentation specifying Luo Han Guo as a primary treatment for headaches; rather, its use is more secondary, targeting the respiratory symptoms that may contribute to sinus discomfort, and thus, indirectly, headache. There is little to no scientific evidence supporting Luo Han Guo for the direct treatment of headaches, sinus or otherwise. Most modern research focuses on its antioxidant and anti-inflammatory properties, and its use as a non-caloric sweetener. Therefore, while Luo Han Guo may be included in some traditional formulas for upper respiratory tract issues that include headache as a symptom, the evidence (both historical and scientific) for its targeted use in sinus headache is weak and largely anecdotal.
