Riboflavin (vitamin B2) has been studied primarily for its potential role in migraine prophylaxis, rather than for tension-type headache. Randomized controlled trials and clinical guidelines have shown that high-dose riboflavin (usually 400 mg per day) can reduce the frequency and severity of migraine attacks in some patients, possibly by supporting mitochondrial energy metabolism in the brain. This has led to its inclusion in some clinical guidelines as a low-risk preventive option for migraine headaches. However, the evidence for its use in treating or preventing tension-type headaches is much less robust.

There is a lack of high-quality studies specifically examining riboflavin for tension headaches. Most available evidence is extrapolated from migraine research, not from trials directly involving tension-type headache sufferers. Therefore, while riboflavin has a scientific basis for migraine prevention, its use in tension-type headache is not well supported by either traditional use or strong scientific evidence. The overall evidence rating for riboflavin in tension-type headache is low (2/5), mainly due to insufficient direct research.
