Arginine orotate has been historically used in some European and alternative medicine settings to support heart health, particularly in cases of "heart weakness" or mild congestive heart failure. The use is based largely on tradition and extrapolation from the properties of its constituents. Arginine is an amino acid known to help produce nitric oxide, a vasodilator that can improve blood flow and potentially support cardiovascular function. Orotic acid (orotate), once considered a "vitamin-like" compound, was believed to enhance cellular energy production by supporting pyrimidine synthesis in heart cells. Early studies conducted in the 1970s and 1980s, mainly in Eastern Europe and Germany, explored magnesium orotate and arginine orotate in cardiac patients, reporting some symptomatic improvements, but these studies were small, not always well-controlled, and have not been widely replicated or cited in modern research. There is little high-quality, contemporary scientific evidence supporting the efficacy of arginine orotate for heart failure or "heart weakness" by current standards. Most references are anecdotal or based on traditional prescribing habits rather than robust clinical trials. Thus, while there is a traditional basis for its use, scientific validation is lacking, and modern guidelines do not recommend it for cardiovascular conditions.
