Asian Plantain (Plantago asiatica) has a long history of use in traditional medicine systems across East Asia, including Traditional Chinese Medicine (TCM) and Kampo (Japanese traditional medicine). In these traditions, the seeds and leaves have been employed to address a variety of ailments, including hemorrhoids. The rationale for its use is primarily due to its purported anti-inflammatory, astringent, and wound-healing properties. Traditional texts suggest that Plantain preparations (such as decoctions, poultices, or topical applications) can help reduce swelling, soothe irritation, and promote healing in the anal region.

From a scientific standpoint, there are some in vitro and animal studies supporting the anti-inflammatory and wound-healing effects of Plantago asiatica extracts, but there is a lack of direct clinical evidence in humans specifically related to hemorrhoids. No high-quality randomized controlled trials have established efficacy for this indication. As such, while the use of Asian Plantain for hemorrhoids is justified by tradition, scientific validation is limited and the evidence is considered weak to moderate (rated here as 2/5), mostly extrapolated from its general pharmacological actions rather than direct clinical results for hemorrhoid treatment.
