Citrus bioflavonoids, particularly diosmin and hesperidin, are used in the management of hemorrhoids with a foundation in scientific research. These compounds are classified as "phlebotonics," which are agents believed to improve venous tone, reduce capillary permeability, and strengthen vascular walls. Several randomized controlled trials and meta-analyses have evaluated the efficacy of micronized purified flavonoid fraction (MPFF), which contains diosmin and hesperidin, for hemorrhoidal disease. For example, a 2012 Cochrane review concluded that flavonoids significantly reduce the risk of persistent symptoms and recurrence of hemorrhoids compared to placebo. Patients reported improvements in bleeding, pain, itching, and overall symptom resolution. The proposed mechanism involves anti-inflammatory effects, inhibition of prostaglandin and free radical production, and improvement of lymphatic drainage. While most evidence comes from studies on MPFF, which is a specific preparation, other citrus bioflavonoid products are sometimes used based on this research. It is important to note, however, that the quality of studies varies, and some products may not contain standardized or effective doses of active compounds. Nonetheless, the use of citrus bioflavonoids in hemorrhoids is one of the more scientifically supported uses for this class of supplements, especially when standardized preparations are employed under medical supervision.
