Synopsis

Source of validity: Traditional
Rating (out of 5): 2

Ruscogenin is a steroidal sapogenin found in Ruscus aculeatus (butcher’s broom), a plant traditionally used in Europe for treating chronic venous insufficiency and related conditions like hemorrhoids. Its use for hemorrhoids is primarily based on folk medicine and herbal tradition, where preparations of butcher’s broom have been administered to reduce swelling, discomfort, and inflammation associated with hemorrhoidal disease. The rationale for this traditional use lies in the plant’s reputed vasoconstrictive and anti-inflammatory properties, which are attributed mainly to ruscogenin and related saponins.

While some laboratory and animal studies suggest that ruscogenin may reduce vascular permeability and improve venous tone, there is limited high-quality clinical evidence specifically for hemorrhoids. One small, open-label clinical trial and several observational studies have examined butcher’s broom extracts (which contain ruscogenin) for venous insufficiency and, by extension, for hemorrhoidal symptoms, with some reports of reduced symptoms such as pain, itching, and swelling. However, these studies often have methodological limitations, such as small sample sizes and lack of controls. No large-scale, placebo-controlled trials have been published specifically evaluating isolated ruscogenin for hemorrhoids.

In summary, while the traditional use of ruscogenin-containing preparations for hemorrhoids is well-documented, robust scientific evidence to validate this application is lacking, and its efficacy remains to be clearly established by modern clinical research.
