Astragaloside IV is a major active saponin component of Astragalus membranaceus, a traditional Chinese medicinal herb. Its use in liver diseases, including hepatitis, is supported primarily by preclinical research rather than extensive clinical trials. Cell culture and animal studies have demonstrated that Astragaloside IV exhibits hepatoprotective effects, such as reducing liver inflammation, inhibiting oxidative stress, and preventing hepatocyte apoptosis. These mechanisms are relevant to the pathology of hepatitis, which involves immune-mediated liver injury and inflammation.

For example, several studies in rodent models of viral or chemically induced hepatitis have found that Astragaloside IV administration led to decreased levels of inflammatory cytokines (such as TNF-α and IL-6), reduced liver enzyme levels (ALT and AST), and improved histopathological features of liver tissue. Some research also suggests Astragaloside IV can modulate the NF-κB pathway, which is involved in hepatic inflammation.

However, while these preclinical findings are promising, human clinical evidence is lacking. No large, high-quality randomized controlled trials have been published to confirm its efficacy and safety in people with hepatitis. Thus, while there is a scientific rationale and supportive preclinical data, the overall level of evidence is limited, and its use in hepatitis should be considered experimental until further clinical studies are available.
