Earthworm (also known as Lumbricus or Di Long in traditional Chinese medicine) is traditionally used in some Asian medical systems, particularly in Chinese medicine, for a variety of ailments, including liver-related diseases such as hepatitis. The historical use is based on the belief that earthworm extracts have properties that can "clear heat" and "detoxify" the body, which are concepts rooted in traditional medicine rather than modern biomedical science. Some traditional texts attribute anti-inflammatory and hepatoprotective effects to earthworm preparations.

From a scientific standpoint, there is limited and low-quality evidence regarding the efficacy of earthworm extracts in the treatment or support of hepatitis. Most of the available data come from in vitro (test tube) or animal studies that suggest possible anti-inflammatory or antioxidant effects, but these have not been translated into robust clinical evidence in humans. No large-scale, peer-reviewed clinical trials have demonstrated clear benefit for hepatitis patients. Therefore, while earthworm is used in traditional medicine for this condition, the scientific validation is lacking, and the strength of evidence is very weak.
