Acemannan is a polysaccharide extracted from the inner leaf of Aloe vera, and has been studied for its potential antiviral, immunomodulatory, and wound-healing properties. Regarding herpes simplex virus (HSV) infections, there is some scientific evidence suggesting acemannan’s potential benefit, although the evidence is limited and primarily preclinical.

Laboratory studies have demonstrated that acemannan can stimulate macrophage activity, increase cytokine production (such as interferon and interleukin), and enhance immune responses, which may help the body combat viral infections like herpes. A few in vitro studies have shown that acemannan may inhibit HSV replication directly or indirectly by modulating immune functions. Additionally, one small, early clinical trial suggested that topical application of an Aloe vera extract containing acemannan might help speed healing of HSV lesions. However, these studies are limited in size, scope, and reproducibility.

No large-scale, high-quality randomized controlled trials have been published to definitively establish acemannan’s efficacy for herpes in humans. Therefore, while the use of acemannan for herpes is grounded in plausible biological mechanisms and supported by preliminary research, the clinical evidence is weak. Most recommendations for its use are based on small studies, preclinical data, and extrapolation from its general immune-modulating effects.
