Phellodendron, particularly the bark (Phellodendri Cortex), has a long-standing use in traditional Chinese medicine (TCM) as one of the "three yellows" herbs, often prescribed for "damp-heat" conditions, including skin lesions and sores. In TCM, recurrent sores or ulcerations, sometimes interpreted as conditions similar to herpes simplex infections, are treated with Phellodendron-containing formulas such as Huang Lian Jie Du Tang. The historical rationale for its use is based on its purported "heat-clearing" and anti-inflammatory properties.

From a scientific perspective, there is limited but emerging evidence that Phellodendron extracts, and their main alkaloids (notably berberine), possess antiviral and anti-inflammatory activity in vitro. Some laboratory studies have demonstrated that berberine and related compounds can inhibit the replication of herpes simplex virus (HSV) in cell culture. However, these studies are preliminary, and there are no robust clinical trials specifically validating Phellodendron’s efficacy for herpes infection in humans.

In summary, the use of Phellodendron for herpes is primarily based on traditional herbal practice, with some low-level scientific support (mostly in vitro). The clinical evidence is insufficient to recommend its use as a primary or adjunct therapy for herpes simplex infections.
