AMP-activated protein kinase (AMPK) is a cellular energy sensor and regulator that has been the subject of significant scientific research in relation to cardiovascular health, including hypertension. Experimental and preclinical studies have demonstrated that AMPK activation can exert beneficial effects on blood pressure regulation. Mechanistically, AMPK activation enhances endothelial nitric oxide synthase (eNOS) activity, leading to increased nitric oxide (NO) production and vasodilation, which can reduce vascular resistance and lower blood pressure. Additionally, AMPK inhibits vascular smooth muscle cell proliferation and reduces inflammation and oxidative stress—factors implicated in the development and progression of hypertension.
Several antidiabetic medications, such as metformin, activate AMPK and have been associated with modest reductions in blood pressure in clinical settings, although their primary indication is not hypertension. Animal studies using direct AMPK activators have consistently shown antihypertensive effects, but human clinical trials specifically targeting AMPK for hypertension treatment are still limited. Thus, while there is a strong mechanistic rationale and supportive animal data, as well as indirect evidence from certain medications, direct, large-scale clinical validation in humans is still emerging.
In summary, the use of AMPK activators for supporting or treating hypertension is scientifically grounded, but the strength of clinical evidence in humans is moderate at present, justifying a rating of 3/5.
