Several clinical and preclinical studies have investigated the effects of chokeberry extracts or juice on blood pressure. Chokeberries are rich in polyphenols, especially anthocyanins, which are believed to contribute to their health benefits through antioxidant, anti-inflammatory, and endothelial-protective mechanisms.

A notable randomized, placebo-controlled study published in 2010 found that daily supplementation with chokeberry extract (300 mg, twice daily) for two months significantly reduced both systolic and diastolic blood pressure in patients with metabolic syndrome. Other smaller studies have reported similar reductions in blood pressure following chokeberry juice or extract consumption, though the magnitude of the effect varies between studies.

The antihypertensive effect is thought to be mediated by improved endothelial function, increased nitric oxide availability, and reduction of oxidative stress. However, most studies have been relatively short-term and involved small sample sizes, so questions remain regarding the long-term efficacy and optimal dosage. Chokeberry is not a replacement for conventional antihypertensive therapies, but the current evidence supports its use as a supportive measure for those with elevated blood pressure, particularly in the context of a healthy diet and lifestyle.
