L-citrulline is a non-essential amino acid that serves as a precursor to L-arginine, which is then converted to nitric oxide (NO) in the body. Nitric oxide is a vasodilator, meaning it helps relax and widen blood vessels, which can lower blood pressure. Several small clinical studies and experimental research have investigated the effect of citrulline supplementation on blood pressure, particularly in individuals with prehypertension or mild hypertension.

A meta-analysis published in 2018 in the journal Current Hypertension Reports concluded that L-citrulline supplementation can modestly reduce systolic and diastolic blood pressure, especially in populations with elevated baseline blood pressure. The reviewed studies generally used doses between 3 to 6 grams per day, and most reported only mild reductions (typically 4-7 mmHg in systolic blood pressure). The evidence base is limited by small sample sizes, short intervention durations, and some heterogeneity in results. There is stronger evidence for blood pressure reduction in specific contexts, such as in patients with endothelial dysfunction or those with higher baseline blood pressure.

Overall, there is modest scientific evidence that citrulline may help support blood pressure reduction, but it is not robust or definitive enough to recommend as a primary treatment for hypertension. Larger, longer-term trials are needed to confirm its efficacy and safety. Citrulline is not a traditional remedy for hypertension, and its use is based on modern scientific investigation rather than historical practice.
