Omega-3 fatty acids, particularly eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) found in fish oil, have been scientifically studied for their effects on blood pressure. Multiple meta-analyses and randomized controlled trials (RCTs) have shown that supplementation with omega-3 fatty acids can result in modest but clinically meaningful reductions in both systolic and diastolic blood pressure, especially in individuals with untreated hypertension. A 2022 meta-analysis published in the Journal of the American Heart Association concluded that higher doses (2-3 grams per day) of EPA and DHA are associated with greater blood pressure-lowering effects. The proposed mechanisms include improved endothelial function, reduction in vascular resistance, anti-inflammatory effects, and decreased blood viscosity.

While some traditional diets (such as the Mediterranean and Inuit diets) high in omega-3-rich seafood are associated with lower rates of cardiovascular disease, the primary justification for omega-3 use in hypertension is based on scientific evidence rather than tradition. Major health organizations, including the American Heart Association, recognize the blood pressure-lowering effects of omega-3 fatty acids, although they may not recommend supplementation for everyone. Overall, omega-3 fatty acids are considered a scientifically validated adjunct to lifestyle modification for mild to moderate hypertension, but should not replace prescribed antihypertensive medications without medical supervision.
