Thyroid (referring to desiccated thyroid extract, typically of porcine origin) has a long history of use in the treatment of hypothyroidism. Its use is primarily justified by scientific evidence. Before the isolation and synthesis of levothyroxine (T4), desiccated thyroid was the standard of care for hypothyroidism. It contains both thyroxine (T4) and triiodothyronine (T3), the two principal hormones produced by the human thyroid gland. Multiple clinical studies and decades of medical practice have demonstrated that thyroid hormone replacement is essential to correct the metabolic deficiencies caused by hypothyroidism.
Desiccated thyroid is approved by the FDA for the treatment of hypothyroidism and is still prescribed, although synthetic levothyroxine is now preferred due to its consistent potency and fewer fluctuations in hormone levels. Randomized controlled trials have shown that both synthetic and desiccated thyroid preparations are effective in normalizing thyroid function tests and alleviating symptoms of hypothyroidism. The choice between desiccated thyroid and synthetic preparations is often individualized, but the overall efficacy in treating hypothyroidism is well established and supported by robust scientific evidence. Major clinical guidelines (e.g., American Thyroid Association) recognize thyroid hormone replacement as the mainstay of therapy for hypothyroidism.
