Thyroid substance, also known as desiccated thyroid extract, is used to support or treat hypothyroidism based on scientific rationale and historical precedent. This preparation is derived from the dried thyroid glands of animals (usually porcine or bovine) and contains both thyroxine (T4) and triiodothyronine (T3)—the two main hormones normally produced by the human thyroid gland. Before the synthesis of levothyroxine (synthetic T4), desiccated thyroid extract was the primary treatment for hypothyroidism for decades, dating back to the late 19th and early 20th centuries.

The scientific justification lies in the fact that hypothyroidism is caused by insufficient production of thyroid hormones. Supplementing with thyroid substance directly addresses this deficiency by providing exogenous T4 and T3. Clinical studies from the mid-20th century confirmed that desiccated thyroid could improve hypothyroid symptoms and biochemical markers. However, more recent research has shown that synthetic levothyroxine is more consistent in dosage and is generally preferred as standard therapy. Despite this, some patients report feeling better on desiccated thyroid, and a few controlled studies suggest that a minority of patients do prefer combination T4/T3 therapy, which desiccated thyroid provides.

Currently, desiccated thyroid is still recognized as a legitimate, though less commonly used, treatment for hypothyroidism, especially in patients who do not respond optimally to levothyroxine alone. Its use is supported by both historical tradition and a scientific understanding of thyroid hormone replacement.
