Hydrogen peroxide (H2O2) has a well-established historical and scientific basis for use as an antiseptic to support or treat minor infections, particularly involving wounds or superficial skin injuries. Its antimicrobial action is due to the release of oxygen when it comes into contact with tissues; this process creates an environment that is hostile to many anaerobic bacteria and helps to mechanically cleanse wounds by effervescence. Numerous in vitro studies have confirmed hydrogen peroxide's broad-spectrum antimicrobial activity, including against bacteria, viruses, and fungi.
However, clinical evidence for its efficacy in treating established infections is limited and mixed. While hydrogen peroxide can reduce microbial load on the surface of wounds, it is also cytotoxic to human cells, including fibroblasts and keratinocytes that are essential for wound healing. For this reason, many guidelines now recommend against its routine use for wound care, especially in deep or chronic wounds, as it may delay healing or cause tissue damage. Its use is generally limited to initial wound cleaning and not for ongoing infection management.
In summary, the use of hydrogen peroxide for treating infections is supported by scientific evidence regarding its antimicrobial properties, but clinical support is moderate (rated 2/5) due to its cytotoxicity and lack of benefit over other safer antiseptics.
