Isoalantolactone is a sesquiterpene lactone isolated from the roots of Inula helenium (elecampane) and other plant sources. There is scientific evidence from in vitro (cell culture) and some in vivo (animal) studies that isoalantolactone exhibits antimicrobial activity against various bacterial and fungal pathogens. Research has demonstrated that isoalantolactone can inhibit the growth of Staphylococcus aureus, Escherichia coli, Candida albicans, and other microorganisms, possibly by disrupting microbial cell membranes and interfering with critical cellular processes. Additionally, isoalantolactone has shown anti-inflammatory and immunomodulatory effects, which may indirectly support recovery from infections.

However, the evidence is largely preclinical and has not been validated in rigorous human clinical trials. While these laboratory findings are promising, they do not provide sufficient support for the use of isoalantolactone as a stand-alone or primary treatment for infections in humans. Traditional medicinal systems have used elecampane root for respiratory infections, but the attribution of this effect specifically to isoalantolactone is not well established in historical records. Overall, the scientific evidence supporting its use for infections is preliminary, justifying a rating of 2 out of 5.
