Peroxidase enzymes, particularly horseradish peroxidase and lactoperoxidase, have been studied for their antimicrobial properties. The scientific rationale centers on the enzyme’s ability to catalyze the oxidation of various substrates (such as thiocyanate or iodide) in the presence of hydrogen peroxide, generating reactive oxygen species and antimicrobial compounds. The lactoperoxidase system, found naturally in saliva and milk, has been shown in laboratory and some clinical studies to inhibit the growth of bacteria, fungi, and viruses. This system is sometimes used in oral hygiene products and as a preservative in certain foods and cosmetics for its antimicrobial effects.

Despite this mechanistic and preclinical support, direct evidence from high-quality clinical trials establishing the efficacy of peroxidase enzymes for treating or preventing infections in humans is limited. Most studies are small, laboratory-based, or focused on in vitro/ex vivo effects, with few well-powered, placebo-controlled clinical trials. Thus, while there is a plausible scientific basis and some supportive preclinical data, the evidence for routine clinical use of peroxidase to treat or prevent infections in humans remains weak to moderate at best. More rigorous human trials are needed to firmly establish its effectiveness in this context.
