Acemannan is a polysaccharide derived from Aloe vera, and it has been studied for its potential antimicrobial and immunomodulatory properties. Several in vitro studies have demonstrated that acemannan exhibits antibacterial activity against a range of Gram-positive and Gram-negative bacteria, including Streptococcus species and Staphylococcus aureus. The proposed mechanisms include stimulating macrophage activation, enhancing phagocytosis, and the production of nitric oxide, which can contribute to antibacterial defense. Additionally, acemannan promotes wound healing, which may indirectly contribute to infection control in cutaneous wounds.
However, most of the evidence supporting acemannan’s antibacterial effects comes from laboratory (in vitro) studies and animal models rather than robust clinical trials in humans. There are limited clinical data directly supporting its efficacy as a primary treatment for bacterial infections. Its use in commercial wound dressings and oral care products is based on these preliminary findings and its safety profile, but it is not recognized as a substitute for standard antibiotic therapies. Overall, while acemannan shows promising antibacterial properties in experimental settings, the current scientific evidence supporting its use in treating bacterial infections in humans is limited, warranting a moderate evidence rating.
