Immunoglobulin G (IgG) is used in clinical practice to support or treat bacterial infections, particularly in individuals with primary or secondary immunodeficiencies who are unable to produce adequate amounts of endogenous antibodies. Scientific evidence supports its use in this context. Intravenous immunoglobulin (IVIG), a purified preparation of IgG derived from pooled plasma of healthy donors, provides passive immunity by supplying a broad spectrum of antibodies that can neutralize bacteria and their toxins, enhance opsonization, and facilitate phagocytosis. Multiple randomized controlled trials and observational studies have demonstrated that IVIG therapy reduces the frequency and severity of bacterial infections in patients with conditions such as X-linked agammaglobulinemia, common variable immunodeficiency, and some hematologic malignancies with secondary hypogammaglobulinemia.
In these populations, regular IVIG infusions significantly decrease the incidence of recurrent, severe, or life-threatening bacterial infections, including pneumonia, sepsis, and sinusitis. However, in immunocompetent individuals or for the general treatment of bacterial infections, IgG is not routinely used and antibiotics remain the standard of care. The use of IVIG for bacterial infections is therefore scientifically validated, but its application is restricted to specific patient populations with compromised humoral immunity. The overall evidence is robust (rated 4 out of 5), based on the consistency of clinical outcomes in immunodeficient patients and the biological plausibility supported by mechanistic studies.
