Pelargonium—particularly the species Pelargonium sidoides—has been studied for its antiviral and immune-modulating properties, especially in the context of respiratory viral infections. Extracts such as EPs 7630 have been the subject of randomized controlled trials and meta-analyses, mostly for acute bronchitis and the common cold. Several studies suggest that Pelargonium extracts can reduce symptom severity and duration in viral upper respiratory tract infections compared to placebo. Proposed mechanisms involve enhancement of host immune responses, inhibition of viral attachment and replication, and mild antimicrobial effects. However, most clinical trials are of moderate quality and focus on symptom relief rather than hard clinical outcomes like reduction of viral load or prevention of complications. Safety profiles in these studies have generally been acceptable, with mild gastrointestinal symptoms being the most common side effects. While the evidence base is not robust enough for unequivocal clinical recommendations, regulatory agencies in some countries recognize Pelargonium extracts as a possible adjunct for treating viral respiratory infections. There is less direct evidence for other types of viral infections, and data outside respiratory illness are limited. Thus, the use of Pelargonium for viral infection is supported by moderate-quality scientific evidence, but primarily for respiratory viruses.
